New Patients

We welcome new patients to Belgravia Medical Centre. As a new patient your appoint will include a visit with both our nurse and GP. Please complete our Personal Information and Consent Form and bring it with you to your first consultation. Alternatively, you may arrive 10 minutes early for your appointment and complete the form at out practice.

Request for Scripts

If you require a new or repeat script, please phone our practice to book in for an appointment.

Referrals

To request a new or repeat referral, please phone our practice to book in for an appointment. Please note that referrals cannot be back-dated.

Medical Certificates

To request a medical certificate please phone our practice to book in for an appointment. Please note that medical certificates cannot be back-dated.

Electronic Communication

Communication via electronic means (i.e. email) is not guaranteed to be secure and there is potential for a patient's privacy to be at risk. There is also the possibility of information being intercepted and/or read by someone other than the intended recipient. We will gain a patient's consent prior to sending any medical information via email.

We appreciate your feedback, and your requests and complaints will be treated confidentially.

Our Practice representative will contact you within a reasonable time after receipt of any complaint to discuss your concerns and your options. If you feel that the clinic cannot resolve your complaint/concern please contact the Officer of the Health Service Commissioner on 1300 582 113.
